Can I just buy the cover and install on my exisiting outlet, or do I need to buy the base also?

You would have to take the outside cover off and put the new in-use box cover over the outlet that is covered by the outside caps and it should fit over your outlet. There are different Inserts with the N use cover to cover regular outlet receptors GFCI receptacles and I think even a single receptacle.

What is the size of the lock? DIameter of the hole for lock?

These don't have an opening for a lock. They do have a snap latch which keeps the lid securely closed. It won't open accidentally.

Does it come in a double gang cover for a quad outlet?
