Would this game be appropriate for a mentally challenged teenager?

yes I think so, the game is very simple and it is a lot of fun it is only a two player game, some of the pictures of the toppings aren't really clear but I think a mentally challenged teenager would have fun with this game!

Is it possible to buy extra dice

The game is only for 2 players. We bought 3 games so we can have 6 people playing together. Don’t know if you can just buy the dice or not but it’s truly not that much to just buy another game or I guess you could take all the cards out that have 5 items to find on them and both just play with 4 dice.

is it too simple for adults only?

While it is fun to play with the grandkids, It does require good hand eye coordination. Learn to play with the simple rules, then experiment with some twists to make it adult interesting
